Appointment Forms
Appointment forms are available online to review and complete in the convenience of your own home. New and returning patients should plan to complete the Registration and Patient History forms. All patients are asked to review the Notice of Privacy Practices (HIPAA). Complete the Medical Records Request form to request medical records from our office. Please fax the completed forms to our office or bring them with you to your appointment.

Assessing BPH Symptoms
Men experiencing urinary voiding symptoms: Please take the American Urological Association's BPH Symptom Index Questionnaire. Please print the results and bring them with you to your appointment. This information is useful to your urologist when evaluating your symptoms.
What to Bring to Your Appointment
- Your insurance card
- Completed forms for your appointment (available online above)
- Physician referral forms if required by your insurance provider
- A list of medications including the dose and frequency
- Pertinent information about your medical and surgical history
- Any recent x-rays or test results related to your condition
Insurance and Payment Information
We participate in Medicare and accept most preferred provider organization (PPO) contracts. We also accept ONLY one HMO, "Santa Barbara Select IPA HMO Insurance".
As a courtesy to our clients, we will bill both primary and secondary insurances. Co-payments are due at the time of service. Credit cards are accepted. Our fees are comparable to other urologists in the Santa Barbara area.
